DentalOne offers you access to high quality affordable dental coverage
for your entire family. Coverage is
provided for preventative, basic and major dental services per insured
person as follows:

Selecting a Plan -
The most popular dental insurance option is the PremierOne
plan. With this insured plan, there are no waiting periods and you
can use any dentist. The PremierOne plan pays 100% of usual and
customary for Preventive
Services:
Cleanings, exams, sealants, fluoride and Diagnostic:
Bitewing X-rays, full mouth X-rays. For Basic
Services: Fillings, extractions, repairs, the
plan pays 25% the first year, 50% year two and 75% year
three and after. For Major Services: Endodontics, Periodontics, Oral
Surgery, Crowns, Bridges, the plan pays 10% the first
year, 20% year two and 40% year three and after.
There is a life time deductible of $100, a $10 office
visit copay and a calendar year maximum of $1,250 per
person. If you need coverage for major services, you
might want to consider one of our discount plans.

Who is eligible for this coverage?
This plan is offered anyone between the age of 18 and 64, and their
eligible dependents (state requirements for dependent eligibility may
vary). Child only coverage is not available.

When does my coverage start?
Coverage starts on the effective date. The effective date issued will
begin on the first of the month (at 12:00 a.m.) following your
enrollment..

What are my payment options?
You can pay Monthly by or credit card or through auto bank withdrawal .
Quarterly or Annually can be paid by credit card.

What is the calendar year maximum?
The maximum amount payable for all Covered Dental Charges in any
calendar year as shown in the Coverage Schedule above. The Calendar Year
Maximum will apply to each insured person
